10th Annual Meeting on Evidence Based Management of Cancers in India
Evidence-based medicine, or EBM, has been defined as “the conscientious, explicit, and judicious use of current best evidence in making decisions about the care of individual patients” and as “the integration of best research evidence with clinical expertise and patient values” in patient management. It is important not only to understand the definition but also the philosophy behind it. The concept of EBM automatically implies that patient care would be improved by following its tenets. Evidence from new clinical research doesn't just invalidate previously held dogmas – it replaces them with management options which are likely to be superior, more accurate, more efficacious and safer.

The major thrust of EBM is to promote the use of high quality, reliable data through a well-accepted hierarchy of evidence. Studies suggest that there is considerable improvement in patient outcomes when treatment decisions are made based on evidence based medicine. It therefore becomes the responsibility of the modern physician to adhere to clinical practice based on EBM and integrate it with his clinical expertise and his patients' values to provide high-quality, holistic medical care. In today's era, this is no longer an individual physician's personal choice; it is a necessary mandate.

The Tata Memorial Hospital has pioneered the cause of EBM in oncology in India and has been conducting the annual meeting on EBM in common cancers for the past nine years. Each year, we have focused on a different aspect of cancer care and collated and published the best available evidence. This meeting helps busy clinicians from all over the country and abroad to get updated on the best available evidence in oncology in a span of 2-3 days, thereby translating into better overall patient care. This year, we focus on Head and Neck cancers, Hodgkin's lymphomas and Infections and cancer.

The meeting is targeted at medical, surgical and radiation oncologists, general surgeons, ENT surgeons, pathologists, radiologists, infectious disease specialists, physicians, basic scientists, researchers and residents in training in any of these specialties. The faculty comprises renowned national and international experts in the respective fields.

  Head and Neck Cancers
   
Head and neck cancers comprise 25% of all cancers in India and are one of the common causes of mortality and morbidity in males. Also most of these cancers are related to the use of tobacco. The other important aspect is that unfortunately, in the absence of an effective screening programme, lack of awareness and ignorance almost 70% of the patients present in advanced stages, making the cure and management of these patients a challenging task.

However, what is encouraging is that there have been improvements in the understanding of the biology of this disease, together with advances in its diagnosis and treatment. Refinements in surgical techniques, radiation therapy delivery and availability of more effective chemotherapeutic agents have made it possible to improve control rates. The use of a multidisciplinary approach in the optimal management of the disease, with integration of supportive care, rehabilitation and quality of life of patients has further improved outcomes.

The present EBM conference aims to address some of these important aspects in the management of this common problem in our part of the world and will evaluate the best available literature in the context of our country.

  Infections and Cancer
   
Nearly 1/5th of the global cancer burden is attributable to infectious agents, with a higher percentage in developing countries. The principal infectious agents associated with cancer morbidity worldwide are Helicobacter pylori , human papilloma viruses , hepatitis B and hepatitis C viruses .A consensus is emerging that efforts to curb infection and infectious disease will also reduce the global burden of cancer. Vaccinations could be key to preventing some of these cancers that are associated with infection

.Cancer has been associated with AIDS even before the human immunodeficiency virus was identified. Three such malignancies considered as AIDS defining are Kaposi sarcoma, aggressive B-cell non-Hodgkin lymphoma and cervical cancer. Several other cancers are seen at increased rates in people with HIV. Many of these tumors are associated with other cancer-causing viruses.

Current evidence and issues regarding biology, therapeutics and prevention of infection related cancers and cancers in HIV infected individuals will be deliberated upon by eminent national and international faculty .

  Hodgkin’s Lymphoma
   
Hodgkin's lymphoma remains one of the most curable of all malignancies across all age groups. Since its first description in 1832, there have been significant advancements in the treatment and outcome of Hodgkin's lymphoma. Current treatment strategies have resulted in long term survivals of above 90 & 60 % respectively for early & advanced stage diseases. Such improvement in outcomes has highlighted the need to devise strategies that would reduce long term toxicities of treatment without compromising disease control.

EBM 2012 will focus on current issues in diagnosis, staging and management of Hodgkin's lymphomas in both adult & pediatric age groups. The scientific program has been structured to highlight not only the currently accepted standard of care but also to address controversial issues and ongoing research areas. The sessions spread over a 3 day period would include a wide range of topics ranging from pathology, the role of imaging modalities like PET scan, the optimum diagnostic pathways, therapeutic algorithm in the management of Hodgkin's lymphomas, late effects of therapy & rehabilitation. This conference is also intended to offer a platform for participants to discuss research areas of mutual interest that could translate into multicentre studies. The invited faculty includes individuals with significant scientific contribution towards defining the current management of Hodgkin's lymphoma.
