Referrals

Information regarding the procedure for obtaining opinion on referral material

Requests for Pathology specimen reporting may be addressed to HOD / consultants .  Samples are accepted at Counter No.37, Ground Floor, Main Hospital Building on all days, except Sundays, between 8am and 7pm and on Saturday between 9am and 2.15pm.

The following details are essential:

• Patient’s name
• Age/Sex
• Full Postal address (reports are routinely despatched by courier)
• Telephone number/ Fax Number/ E-mail address (if available)
• Clinical details
• Site of disease, nature of material and details of previous surgery/ biopsy
• Radiological images with radiology reports.  These are essential for Bone soft tissue and CNS cases
• Results of hematological and biochemical tests, including tumour marker levels if relevant

The physician requesting the tests may please forward his complete address, contact number and e-mail ID to assist the reporting process

A single demand draft for each patient may be drawn in favour of “Tata Memorial Hospital, Department of Pathology.” There are no extra charges for Immunohistochemistry.

Separate demand drafts are required for each case referred.

Slides/ blocks prepared elsewhere and submitted for opinion are retained by the Department.

On request, reports can be forwarded by Fax or E-mail.
